“I think there is a risk of shortening pins of the DSI if the heatsink is slightly tilted.Likely, this happen to me. Placing insulating tape on the edge of the heatsink might be a solution.”
“Although it works “ok” it could be better. The fins need to be longer for the pi4 which runs hotter. My one (@ 100% cpu running SETI) is still thermal throttling without a fan. Also the thermal tape is not, it’s just adhesive tape, it needs to be thermally conductive to work better. I will experiment with thermal glue and pastes.”

I've seen kits for the RPi4 from other vendors and they look more appropriate in having three sinks to apply to three chips (though they don't come with instructions, either!)
This sink is a bit big and so easy to accidentally lever off the CPU if pushed while handling the RPi.
