It comes with pre-attached thermal tape underneath, therefore it's ready to use.
I've had a problem in that the size of the base is slightly larger than 40mm and, possibly, my RPi has a low tolerance between CAMERA and DISPLAY sockets.
Simple solution: rotate the heatsink making sure it doesn't touch anything else that the CPU it is placed on.
